• Complete affidavit form for each prize
• US residents only: Please complete substitute W9 form

Your list of prizes includes the prize won, value, and statuses. The prize value reported is the cost of the item at the time it was purchased.

Prize redemption restrictions - Please note expiration dates for each prize. If you decline, or do not complete all of these steps before the expiration date your prize will be awarded to the next qualified participant.

Note to US residents : Taxpayers must report the fair market value of merchandise or products won as a prize or award, as taxable income if totaling $600 or more. The Substitute W9 will help you to complete the necessary tax return forms in order to report your prizes as taxable income.
